Having good data to understand and plan for the needs of the population (whether at a community level or individual level) means that we can reduce inequalities and improve outcomes for all people.
Overview
Digital technology can be used to better address inequalities and improve outcomes for all people. Digital technology and better use of data insights can facilitate support being targeted and driving improvements to the health and wellbeing of the populations that providers serve.
A key element of this success measure is a provider’s ability to work with partners in the local health and care system, including the local council where appropriate. This may mean safely and ethically sharing and receiving information about the people drawing on care to improve population health management as well as commissioning and care outcomes (for example in relation to equality, diversity and inclusion).
Providers might also use uses guidance such as that found on the information governance portal and Better Security, Better Care Programme to decide upon the correct and lawful way to use data. It is essential that personal data is used appropriately and safely for service improvement or commissioning purposes.

- Data for People - Think Local Act Persona (TLAP)
- TLAP got together with people who draw on care and support and family and friend carers to co-produce 15 key principles and 6 actions relating to the collection, storage, sharing, and use of data in care in England. The principles and actions identified relate to themes such as commitment to co-production, equity and inclusion, and trust and transparency.

- 'Personalised Care and Support Plan' Standard - Professional Records Standard Body
- Personalised care planning standards will help people manage their own care, with the support of a wide range of services including GPs, hospitals, occupational therapy and social care. This standard will help them and the health and care professionals who support them to get the right information when they need it.
- VIVALDI Social Care - UCL, The Outstanding Society and Care England
- A new partnership with the aim of reducing the impact of infections and outbreaks in care homes. Together with care home staff, residents, family members and providers, VIVALDI have coproduced a plan for a 12 month pilot study. The study will use data that the NHS and public health agencies routinely collect to measure levels of common infections in care home residents and will generate an anonymised database that can be used for research.

- The BCF Support Programme 2023-2025 is a partnership between the LGA, ADASS and Newton Europe.
